The IVF Puzzle and its Complication

In Vitro Fertilization (IVF) is a medically assisted procreation method in which a man’s sperm and women’s eggs are combined outside the body in a laboratory. IVF has proven safe over time. The likelihood of something serious happening to you or your baby is very low. However, in very rare cases, there may be complications that can be serious. Medication Side Effects: Medications used for ovarian stimulation have mild side effects for some women, including bruising and mild pain at the injection site, headaches, upset stomachs and mood swings. Another side effect is ovarian hyper-stimulation syndrome (OHSS) and this occurs when the ovaries produce many follicles and fluid can flow from the blood vessels into the abdominal cavity and lungs.

Egg Retrieval: Once the eggs have matured, the patient goes to the fertility center for egg retrieval, usually 10 to 14 days after the start of the cycle. During the breeding season, mild to moderate discomfort can usually be felt during or after the procedure. In rare cases, there may be excessive bleeding of the ovaries or injury to organs near the ovaries, such as the bladder, intestines or blood vessels. Pelvic infection is another risk.

Embryo Transfer: When the embryos are transferred, a speculum is placed in the vagina and the embryos are placed in the uterus using a catheter. Some women experience mild cramping when the catheter is inserted into the cervix.

The biggest worry in every IVF patient’s mind is the possibility of a mix up in their eggs and / or sperms and /or embryos with someone else’s, meaning that the baby that they are carrying is not their own. When a woman naturally conceives, the fertilization occurs inside her body and there is a guarantee about the genetic materials that has made the baby. In IVF, the fertilization occurs outside the body and the embryo is implanted surgically into the womb. In this situation people have the rights to get a DNA test (Paternity/Maternity/Paternity Trio), in which swabs from the mouth of the baby and the parents are taken and verified that baby is biologically related or not.

Birth Defects: The congenital abnormalities can occur in advanced maternal age of women, abnormal Ultrasounds, personal or family history of aneuploidy, previous pregnancy with a chromosomal condition or during receiving IVF treatment /Donor eggs. Naturally-conceived babies have a 2-3% chance of having birth defects, while IVF-conceived babies have an estimated risk of 2.6 to 3.9%. Risk increases when woman falls into the older age group or any of parents have genetic disease history. In these cases Non Invasive Prenatal Test (NIPT) could be blessing and beneficial for patients or doctors.

NIPT is a new prenatal screening test that provides pregnant women with accurate information about their baby’s genetic health without the risk of invasive testing such as amniocentesis or chorionic villus sampling. It also detects Whole Genome – 23 pairs of human chromosomes, Common Chromosomal abnormality: Trisomy 13 (Patau’s Syndrome) Trisomy 18 (Edwards’ Syndrome) Trisomy 21 (Down’s syndrome), Microdeletions – 20 specific diseases – DiGeorge syndrome, Williams syndrome, Prader-Willi syndrome, Angelman syndrome, Hereditary Neuropathy with Liability to Pressure Palsy (HNPP), Rubinstein-Taybi syndrome, Miller-Dieker syndrome and many more.

Miscarriage and Ectopic Pregnancy: The miscarriage rate after IVF is similar to that of the natural conception, about 12-15% for women in their twenties. The risk increases with the age of the mother in both cases and can go up to more than 50% for women of 40 years.

Ectopic pregnancy is done when the embryo implants in the fallopian tube instead of the uterus, this is rare with IVF. This is a potentially dangerous medical emergency that must be treated quickly to end the pregnancy and prevent serious health problems or even death.
